HOPE is committed to protecting the privacy of members. As required by law, we treat all health information confidentially and is HIPAA compliant. Autism HOPE has developed an internal practice to ensure the privacy and confidentiality of your health information.
01. Our Legal Duty
We are required by law to protect your health information. If there is a security breach of your protected health information (PHI), in most instances, we are required by law to notify you. We are also required to give you this Notice about our privacy practices, our legal duties and your rights concerning your health information. We will follow the privacy practices that are in this Notice while it is in effect.
We do not provide marketing services for the advertiser whose site you visited as a result of using a link to another site from our website. We do not and are not responsible for third-party websites. If you click on one of the online links, you may be directed to a landing page or website not hosted by us ("Hosted Site").
If you visit a Hosted Site resulting from a link, and that site has a lead form, that lead form may be emailed directly to advertisers. By doing this, you are providing your private information to that advertiser who may subsequently call or email you as a result.
Our services are stricly for the benefit of our members. Our website does not collect "cookies" and we do not track your browsing history when you visit our website or our practice. Any phone number or email on our website is provided only as a benefit for the education/information of our members and website visitors.
02. What Informaton Do We Collect?
If you complete the contact form on our website and click the "Submit" link, that information is collected. We may use the information you provide to contact you regarding our services. We keep this information in our member account to assist us in assessing your needs.
If you contact us by calling a telephone number that we have provisioned for our website or mailing information packet, we may collect PII such as your name and telephone number. We do not record the telephone conversation or track the performance of our services for quality assurance purposes. If the telephone conversation will be recorded, you will be notified before the conversation begins.
03. How We May Use Your PII
We may use or disclose your health information for Treatment, Payment and Health Care Operations Activities ("TPO") purposes without your written authorization. This means that those who are involved in your child’s care and treatment will have access to your health information. In order for us to receive payment for the care we provide, we will tell your insurance company about that care. We may also use your child's health information for our own purposes, such as monitoring, planning and developing our care services, and educating our staff.
If you provide us with PII, we may retain the information to assess the patient file. We also may use PII to ensure compliance with our policies and applicable law. We will not sell or share your PII with any affiliated or unaffiliated third parties for marketing purposes.
We will not use your PII for any future communication with you, via email or otherwise, unless otherwise instructed by you.
04. Collection of Personal Information
Any PII that you provide may also be kept and maintained by us and is subject to our privacy policy. If your information is shared with other medical providers, we do not control the manner in which the that provider uses any PII provided to them by us.
Therefore, any questions regarding the treatment of any such information by the other service providers should be referred directly to the provider.
05. With Whom We May Share Your PII
We do not share your PII with our advertisers, as described above. We may, however, use other businesses to perform certain services for us, and PII may be provided to those businesses when that information is necessary for them to complete a requested transaction or otherwise perform their duties. We will take reasonable steps to ensure that these third party service providers are obligated to protect PII on our behalf.
We provide health information from your medical records for the TPO activities for another healthcare provider agency that is not affiliated with us. We would release information about you only if it were needed in conjunction with care services that have been or will be delivered to you (including payment for such care services).
Except as described in this policy, we will not share the content of the emails or the phone conversations with anyone other than the providers as instructed by you.
